Chibi Chibi is a small girl approximately the size of a toddler who only appears in the last arc of the Sailor Moon series.
The anime and manga have drastically different versions of this character.

In the anime Chibi Chibi is only able to repeat her name ("Chibi Chibi") or what other people around her say.
In the ANIME Chibi Chibi is not a person, she is an object. A star seed to be accurate. Her first form is that of a starseed, then a butterfly in which Galaxia has expelled her starseed from her body in order to keep it from being tainted by the Chaos inside her. She calls it the Light of Hope and tells it to find a new suitable host.
When the starseed finds whom it wishes to be it's new host, it takes on the form of a little girl who looks similar to the new host in order to get close to her.
Other forms in which Chibi chibi takes are that of a small senshi similar to Eternal Sailor Moon's form(Contray to rumor, she is not called Sailor Chibi Chibi Moon, her title in this form is Sailor Chibi Chibi), a sword and a shadowy figure of what we can assume Eternal Sailor Moon would have become if she had taken ChibiChibi as her new starseed.
In the MANGA Chibi Chibi is the disguised form of Sailor Cosmos, Eternal Sailor Moon's far future self. Her manga counter-part speaks more and doesn't have the childlike mentality of her anime-counterpart because she is not a child but a grown woman in disguise.
In the manga, Galaxia's star seed in the Saffir(Sapphire) Crystal. A crystal of destruction.
This childlike disguise was conjured for the sole purpose of getting closer to her past self, because as stated in the manga, "She knew herself here was suffering as well, so she stayed close to her and comforted her in hopes she would make the right decision." This decision refers to Sailor Cosmos's regret. This regret stems from when Sailor Cosmos was Eternal Sailor Moon, and did not kill Chaos but trapped him instead allowing him to become Sailor Chaos in the far future. She had hoped to change this event by influencing her past self to destroy the Galaxy Cauldron this time around instead of letting Chaos live inside it.

- Chibi Chibi is often mistaken to be Usagi's granddaughter, but in the manga, Chibiusa clearly stated that Chibi Chibi cannot be her daughter. This is also proven true, because in the manga, Chibi Chibi is Sailor Cosmos, the ultimate form of Sailor Moon. In the anime, Chibi Chibi is Sailor Galaxia's star seed.
